T-Ball Registration (This is the form to use if you have a girl that wants to play in our recreational T-ball league.)

Softball Registration (This is the form to use if you have a girl that wants to play in our recreational softball league.)

Sponsorship Letter (This is a letter for previous sponsors about the upcoming season)

Sponsorship Agreement (This is the form you will need if you would like to become a sponsor of Sedalia Girls Softball.)

Coaches Code of Conduct (ALL coaches and assistant coaches must read and sign this form before they will be allowed to coach.)

Criminal Background Check (ALL coaches and assistant coaches must pass a background check.)

Scholarship Application (Form to fill out to apply for our $500 college scholarship.)

The scholarship application are due by June 15th any received after that date will not be considered.
